S. A. General Hospital, West's Building, General Hospital, Baltimore, Maryland September 25, 1863 [note, there is a record of a Thomas O. Teank, captured at Gettysburg, July 5, 1863 which the War Dept. 12/22/1910 believed in error and that of T. O. Servis), transferred December 17 to Marine Hospital, Anne Arundel County, Md., slight leg wound due to Cannister & Shell wound, captured July 20, 1863 at Martinsburg, Virginia, died December 29, 1863, malignant smallpox, a number of documents regarding inventory of his effects, record he "died in the Marine Hospital, Baltimore "
